The patient portal can be accessed by going to cascade.ema.md and selecting Patient Portal.
What is my username?
In general, the username is your email address unless:
- You have been provided a unique username for your account
- You are a proxy user for a family member
How do I access FollowMyHealth?
Access to your previous portal through FollowMyHealth has not changed. However, we stopped adding information to that portal in March of 2021. If you have been to our practice since March of 2021, please visit our new portal at cascade.ema.md to access your information.

Accessing the patient portal works best on Google Chrome or Mozilla Firefox.
Verify the portal you are trying to access is cascade.ema.md and not the previous portal FollowMyHealth as referenced above.
If you forgot your password, use the reset password link.
